Completing a referral form is the first step in the process of accessing any of the services offered at The Neurodiversity Clinic. On the referral form, you will be asked to provide some basic information about what you are looking for and about how those services might be paid for. After we receive the completed referral form, we will use that information to determine if and how we may be able to assist you. Filling out the referral form does not obligate you to access the services you are inquiring about. The process is informational only.
